Safe Patient Project: To Err is Human-To Delay is Deadly

  1. 10 years after the landmark "To Err is Human" report published by the Institute of Medicine, Consumers Union believes that preventable medical harm still accounts for 100,000 deaths each year (a million lives over the past decade).     I spent a couple of years with WHO's Patient for Patient Safety, so thanks for your post. I was very impressed (still am) with a number of patients and professionals who continue to strive for improvements. Best wishes as patient safety is obviously so very important.
