Insofar as Berwick is a radical, he's a radical in believing that vastly more power has to be devolved to the judgments, preferences and desires of patients. When a patient wants someone in the room and the doctor doesn't, Berwick believes the patient should win.

wow....excellent article! now we are called "extremists" for wanting choice? many things pushed at us in this sometimes strange society we live in are often justified as giving people "choice" (not real choice of course, just perceiving choice - huge difference).
