.....The goalpost for universal screening for LS has shifted from
“should we” to “how.” Its feasibility has been well demonstrated in
research and clinical settings, but at the population level, significant
challenges and barriers for successful real-world clinical implementation
remain.22 From the public health vantage point, large-scale universal
screening efforts are advantageous with respect to cost and
quality but would first require coordinated efforts in educating all key
stakeholders and ensuring that public concerns and fears are addressed.
There is an urgent need to establish 21st century infrastructure
(eg, registries, personnel, and so on) in parallel. Like most
worthwhile dreams, if population-based universal LS screening is to
reach fruition, then sweat, determination, and sheer hard work is
required among disparate groups, all of whom, ironically, want the
same end point: high-quality health care at value."
