Scholarships available to e-Patients for palliative care summit – Ignite. Inspire. Innovate


The Coalition for Compassionate Care of California (CCCC) is inviting e-Patients to apply for a scholarship to their 8th Annual Summit in Newport Beach, California, on May 12-13, 2016.
The deadline to submit an application is March 10, 2016.
The e-Patient scholarship program is funded by the John and Wauna Harman Foundation, and will cover the full Summit registration fee. Travel and hotel accommodation scholarships will be awarded on an as needed-basis.
Now in its 8th year, the Coalition for Compassionate Care of California Annual Summit is designed for physicians, nurses, social workers, nursing home administrators, chaplains, patients and consumers who are interested in best practices, quality and policy as it pertains to palliative and end-of-life care. In 2016 we are pleased to announce a special pediatric track that will run throughout the event.
This year’s Summit “Ignite. Inspire. Innovate. Taking Palliative Care to the Next Level,” includes the following topics: Palliative care, patient-centered care, advance care planning, managing serious illness for the adult and pediatric populations, end-of-life care, cultural congruency, and Physician Orders for Life-Sustaining Treatment (POLST). The Coalition for Compassionate Care of California is an interdisciplinary collaborative of thought-leaders from healthcare systems and organizations, government agencies, consumer organizations, and the general public. Our goal is to transform healthcare so that medical care is aligned with individual patient preferences—that people get the care they need and no less, and the care they want and no more. We want to create a community where people have support to explore their wishes for care during a serious illness, express their wishes, and have these wishes honored.

Why include e-Patients at the Annual Summit?

It has been said that patients are the most under-used resource in health care. CCCC is ready to engage the public in advance care planning on a wider scale, and we feel the time is right to establish a relationship with a community of people who are already highly engaged in their own health care. Patients and families offer valuable insights and experience and serve as messengers and champions, not only among e-patients but among the general population.
What is an e-Patient? An e-patient is a health consumer who participates fully in their medical care. Sometimes referred to as an “internet patient,” e-patients see themselves as equal partners with their doctors in the healthcare process. Learn more at e-patients.net.
CCCC commits to creating a welcoming and comfortable environment in which e-Patients will receive the same level of respect and inclusion given to clinicians, researchers, policy makers and other Summit attendees.

Roles and responsibilities of e-Patient delegates

Scholarship recipients are expected to attend the Summit in full, arriving on time and staying until the Summit ends on Friday afternoon, May 13, 2016.
CCCC expects e-Patients to participate at the same level as all other Summit attendees by attending breakout sessions and “office hours,” and encourages e-Patients to network with others.

Travel, hotel accommodations, and dining

Reduced hotel rates are available, and rooming with another scholarship recipient is recommended to reduce costs. Breakfast and lunch are provided at the Summit venue with reasonable accommodations made for dietary needs. Dinner is not provided. The Summit hotel is within 1 mile of several restaurants and a grocery store. Additional consideration should be given to any lost wages from time taken off from work.
Travel and accommodation scholarships may be awarded on an as-needed basis. Please indicate in the application if you intend to apply for a travel and/or accommodation scholarship.
