Conclusions
This study demonstrates that primary
management of patients with adhesive-SBO by medical services is
associated with longer LOS, larger cost, and higher readmission rates,
as well as a delay in time to surgery and higher mortality rates
following surgical intervention. While complex patients with multiple
comorbidities may benefit from medical optimization by hospitalists,
institutional policies favoring primary management by surgical services
may greatly reduce healthcare utilization and improve outcomes for
patients admitted with adhesive-SBO.
