Oncologists' views about the treatments and care associated with advanced ovarian cancer
Background
Ovarian cancer treatment depends on histological type, grade and stage of the tumour and general performance status of the patient. Patients may have different expectations about treatment and place different values on outcomes from those of healthcare professionals.Method
We used an online survey to examine: how UK oncologists manage women with advanced ovarian cancer; available anti-cancer treatments used in practice; oncologists' views on patients' worst symptoms and available supportive interventions. In addition, we are currently interviewing 200 patients about their experiences of treatment and management of their disease in the ADVOCATE study (Advanced Ovarian Cancer: Care & Treatment Experiences).Results
66 oncologists (44 medical; 22 clinical) completed the survey between October and December 2011. 51 worked in a designated gynaecological cancer centre, 54 were NHS Trust clinicians; 59 trialists and 52 respondents treated >31 advanced disease patients annually. Abdominal pain/swelling was ranked the most common presenting feature (n=46). Carboplatin with paclitaxel (n=56) was the usual 1st line treatment and symptomatic disease progression was main trigger for changes in management. Carboplatin with paclitaxel was the most used for 2nd line (n=39) and subsequent treatments in partially/ platinum sensitive patients (n=25); liposomal doxirubicin for platinum resistant/refractory disease (n=42). The majority (n= 63) conducted routine clinical follow-up, measuring CA125 levels regularly (n=43). Bevacizumab was rarely offered as maintenance therapy (n=3), mainly as not NICE approved (n=49). The minimum PFS gains most oncologists (24/50) felt worthwhile for maintenance therapy was 4-5 months but (26/50) believed patients would consider 3-4 months worthwhile. For OS, 36/60 oncologists indicated 3-4mths as worthwhile; 25/60 felt 1-2mths would be their patients' choice.Conclusion
These results reflect current practice of trial-active oncologists when treating advanced ovarian cancer. "In the past five years, there has been a huge advance in our understanding of the biology of the group of diseases collectively known as ovarian cancer. However, these advances have yet to translate themselves into improved outcomes for patients: Throughout the world, survival rates have improved little since the introduction of platinum-based chemotherapy thirty years ago. In the UK, for example, five year survival rates have increased by less than 4% since 1995.
One of the key changes is the realisation that ovarian cancer is not one disease, but many, each with its own distinct aetiology and molecular biology, and collectively linked merely by a shared anatomical location. This realisation is starting to manifest itself in changes to clinical trial design, as subtype-specific studies emerge, utilising novel agents targeting subtype-specific pathways. Further refinement will require international collaboration, especially in the rare subtypes, as well as improved preclinical modelling: We still await a reliable and realistic transgenic model of high grade serous disease, whilst many of the cell lines in widespread use are of uncertain provenance.....

Conclusion
While specificity of transvaginal ultrasound screening was high,
sensitivity was higher for primary ovarian/tubal cancers than for
invasive epithelial cancers alone. While 15 operations were done overall
for each primary ovarian/tubal cancer detected, 25 were done per
primary invasive epithelial cancer. The mortality impact of screening is
awaited in 2014.

Abstract
Inhibitor of IÎşB kinases (IKK) are key regulators of NF-ÎşB signaling. Three IKK isoforms—α, β, and ε—have been linked to oncogenesis, yet the precise components of NF-ÎşB signaling in ovarian cancer have not yet been dissected. We surveyed 120 ovarian cancer specimens for IKK-ε expression. Notably, cytoplasmic expression was elevated in metastatic lesions relative to primary tumors (P = 0.03). Therefore, we hypothesized that IKK-ε drives ovarian cancer metastasis. Conclusion
The age
incidence of ovarian cancer in our patients is 10 years younger than
that seen in Western countries. EOC constitutes the majority among all
ovarian cancer cases, followed by border line tumors. Papillary serous
cystadenocarcinoma predominates other types of EOC. The response rate of
EOC to first line chemotherapy was high but the progression free
survival was lower than that seen in the literature.

The FDA says it recommended the recall "out of an abundance of caution" after its ongoing inspection "raised concerns about a lack of sterility assurance" for Ameridose products.
Despite the recall, the FDA is not asking doctors, clinics, and hospitals that used Ameridose products to warn patients that received the medications. It's simply asking Ameridose customers to stop using the drugs and to return them to the company....

Program BriefThis program brief summarizes findings from Agency for Healthcare Research and Quality (AHRQ)-supported research projects focused on cancer in women published January 2008 through December 2011. " Data from the National Cancer Institute indicate that 1 in 72 women born today will be diagnosed with cancer of the ovary at some point in their lives. Ovarian cancer occurs in white women (12.6 cases per 100,000 women) more often than in women of other races (11.2 Hispanic, 9.3 black, 9.2 Asian/Pacific Islander, and 9.0 American Indian/Alaska Native). According to the Centers for Disease Control and Prevention, 21,204 women were diagnosed with ovarian cancer in 2008, and 14,362 women died from the disease. The overall 5-year survival rate for women diagnosed with ovarian cancer was 44 percent in 2008. More than 90 percent of those diagnosed with localized ovarian cancer survived for 5 years or more, compared with just 27 percent of those whose cancer had spread to distant organs." |
